"REFER A NEW SMILE" PROGRAM

At Dr. Hayosh's office we are grateful for your confidence and trust in us, and we appreciate it when you recommend us to friends and loved ones for their orthodontic care. In recognition of your support, we have instituted the "Refer a New Smile" program. This is our way of saying "Thank You."

The program is simple and easy:

1. Pick up referral cards from our office to distribute to your family and friends who are in need of orthodontic care or an evaluation. On the back of your “Refer a New Smile” card there is a place for both your friend’s name, as well as yours.
2. Fill out the card and pass them out to your friends and family who are not currently a part of our practice.
3. When your referral comes in for their complimentary initial exam appointment, we will reward you with 20 wooden nickels, which can be redeemed for a prize or cash at the end of your treatment. If you are not currently in orthodontic treatment, you will receive a $15.00 gift card. Your referral must present the referral card when they come to the office. (Please keep in mind that the new patient can only bring in one "Refer a New Smile" card.)
4. Once your referral actually begins orthodontic care at our office, we will reward you further with a $25.00 gift card.

APPLE NANO iPOD

We will have two drawings per year for an Apple 4gig Nano iPod to further thank those who have referred patients. The winner will be selected randomly from the referral cards. The more friends you send our way, the better your chances are at winning! The drawings will be held every November and May.
